Incident Recording in Primary Care – Survey

The Patient Safety Team are carrying out a survey to help them better understand incident recording in Primary Care.

Background

Primary care see over 307 million patients per year, nearly double the numbers seen in secondary care (25 million A&E visits and over 123 million outpatient appointments – data from NHS Providers website). However, only a tiny fraction (0.7%) of the two million patient safety incidents annually recorded nationally come from primary care. This highlights there is a significant opportunity to increase incident recording and so increase the chance to gather themes and to identify new or under-recognised patient safety issues and therefore act to prevent future harm to patients.
