Active Herts Project
Active Herts is a new £1m programme targeting the most deprived and least active areas of Hertfordshire.
The project covers four localities including;
The project aims to target inactive adults with risk factors for cardiovascular disease and moderate mental health conditions and support them to become physically active and improve their lifestyle.
What will the project offer?
In each of the four localities a Get Active Specialists is based within selected GP surgeries and Mind Wellbeing Centres to provide one to one consultations for inactive adults. The Get Active Specialists use an evidence based approach through use of a brief intervention using motivational interviewing and applying behaviour change theory.
How to access the project
GP’s and other healthcare professionals can refer eligible patients to the service. Individuals can be referred if they meet the following essential criteria:
- Are inactive doing less than 30mins of physical activity or sport per week
- Are aged 16 years or over
- Live in the boroughs of Broxbourne, Hertsmere, Stevenage and Watford.
Individuals that have one of the following medical conditions in addition to the criteria above can also be referred:
- High Cholesterol
- Diabetes Mellitus
- History of heart disease
- Family history of heart disease
