Hospital discharge: medicines concerns and feedback

If any community pharmacies have any concerns/issues with hospital pharmacies particularly in relation to medicines at discharge please could we request specific examples are fed back to the LPC office. Without this information we are unable to highlight them as issues to the relevant hospitals and commissioners where this is an ongoing problem. Please give as much information as possible, i.e. times/dates, detail (without providing patient and data sensitive information). Please send to info@hertslpc.org.uk
