Hertfordshire LPC Community Pharmacy Awards Night + 2020 and Beyond

We are inviting all community pharmacies across Hertfordshire to take part in the Hertfordshire LPC Community Pharmacy awards 2018/19.

The awards will be presented as part of our AGM on Thursday 4 July 2019 and are based on delivery during 2018/19 (1 Apr – 31 Mar).

The following awards are being decided upon and there are three awards for community pharmacy to make their nominations:

  • No1 – Most Engaged Community Pharmacy for Diabetes Pilot Service (based on figures and judges to review)
  • No2 – Public Health Community Pharmacy of the Year (based on the figures of the top 10 and judges to review)
  • No3 – Community Pharmacy Team Member of the Year – (community pharmacy are invited to nominate and judges to review) – Here is the nomination form
  • No4 – Community Pharmacist of the Year – (community pharmacy are invited to nominate and judges to review) – Here is the nomination form
  • No5 – Advanced Service Delivery of the Year (based on advanced services, MUR/NMS/FLU/NUMSAS figures of the top 10 and judges to review)
  • No6 – Innovation in Community Pharmacy – (community pharmacy are invited to nominate and judges to review) – Here is the nomination form

Please share the nomination forms with your teams as you are welcome to submit multiple nominations. Completed forms MUST be returned to the LPC office by Monday 03 June 2019 at 9am,  when they will be given to the judging panel to select the top three of each award. Any forms submitted after the deadline date will not be included.

All completed forms are to be sent to the LPC office, info@hertslpc.org.uk.  There will be four judges: one from PSNC, one from the LPC office, one nominated from Public Health Hertfordshire and one nominated from NHS England.

We encourage all community pharmacies to sign up to attend the evening. View the agenda and you will see we have engaging speakers and a very full evening so please email the name(s) of who is attending and your pharmacy name to info@hertslpc.org.uk.
