Discharge Medicines Service (DMS) – Information from Lister Hospital on stopped medicines

Please be aware that due to the IT system, Lorenzo, used by Lister Hospital they cannot extract the stopped medication data from Lorenzo because of the way that it is used.

Lister Hospital is aware of this and community pharmacies do not need to inform them. They are working on a different way of providing this information to be available to community pharmacies as part of the minimum dataset for DMS.
