Hospital Discharge Medicine Information Webinar (West Herts Hospital Trust)

West Herts Hospital Trust will be providing electronic discharge information to community pharmacies from Watford Hospital in November 2020 via PharmOutcomes. Locally this is often known as the Electronic Medicines Optimisation Pathway (EMOP) which is already live in Lister Hospital in Stevenage and Princess Alexandra Hospital in Harlow. This is also sometimes referred to as TCAM (Transfer of Care Around Medicines).
CPH are offering a webinar, in conjunction with Watford Hospital, for all community pharmacies to become familiar with the system before its launch in November/December 2020. The event is open to all pharmacies across Hertfordshire who may wish to remind themselves of how the system works in order to interact with these notifications via PharmOutcomes.
