The EMOP project is aimed to be implemented by Lister Hospital on 3 April 2018.
The EMOP project is the electronic and secure transfer of a copy of a patient’s discharge prescription (TTA) from the hospital to the patient’s chosen community pharmacy using PharmOutcomes. On discharge patient’s medicines information from TTA will be sent to the community pharmacy via PharmOutcomes if the patient consents.
Community pharmacies will automatically be opted in to receiving this information via PharmOutcomes for their patients where consent is obtained. It is therefore essential that all community pharmacies and their staff understand what this project is about and how they can use this information so please attend one of these training events.
We strongly encourage one representative from each community pharmacy that has patients that attend Lister Hospital to attend one of these events. Further details here
