Electronic Medicines Optimisation pathway (EMOP) Launching from Princess Alexandra Hospital, Harlow

The EMOP service is the electronic and secure transfer of a patient’s discharge information from the hospital to the Patient’s nominated pharmacy through the PharmOutcomes platform, if consent is provided by the patient.

This will enable you to provide better care and have the knowledge of changes to your patients’ medication.

The EMOP service is going live at Princess Alexandra Hospital, Hospital in the New Year

Community pharmacies will automatically be opted in to receiving this information through PharmOutcomes, if patient consent is obtained. It is therefore essential that all pharmacies and their staff understand what this service is about and how they can use this information.

We strongly recommend at least one representative from each community pharmacy attend this meeting.
